Unity Church Commitment Reservation

We're glad you've chosen Unity Church of Clearwater for your special day. Please fill out the following form to get things started.

Please make sure to call our office at (727) 531-0992 to make sure your preferred date & time are available.

Ceremony times other than the one's listed can at times be accomodated.
An additional fee may apply if you choose a time outside our normal ceremony times.

Please see the document titled "Wedding/Commitment Guidlines" to determine which of our ceremony packages will be right for you.

We require a $100.00 deposit to hold a date for you. If you would like to use a credit card, please stop by the church to place your deposit. If you prefer to use a check, you may mail it to:
